 ips showing income (T4\, T4A\, T4E\, T4RRSP\, T3\, etc.)</p>\n<p>- Final m
 unicipal property tax bill (for each applicable tax year) or rent receipts
  for each address you have lived at during the tax year(s)</p>\n<p>- Notic
 e of Assessment or tax return letter from the last tax year you filed a ta
 x return (if available)</p>\n<p>- For students - tuition receipt (T220A or
  TL11A)</p>\n<p>- For dependent children - receipts for childcare and soci
 al insurance number</p>\n<p>- Secure Certificate of Indian Status (if appl
 icable)</p>\n<p>- Canada Revenue Agency letter indicating approval of the 
 Disability Tax Credit (if applicable)</p>\n<p>- Letter from physician indi
 cating you have a dependent with a physical or mental infirmity\, stating 
 when the impairment began and how long it is to last (if applicable)</p>\n
