 re Planning&nbsp\;at the <a href="https://www.guelphpl.ca/en/about-us/main
 .aspx" target="_blank" rel="noopener" title="Main Library location and hou
 rs">Main Library</a>.</p>\n<p>Do you know who will make decisions for you 
 if you are unable to speak for yourself?&nbsp\; This question is crucial f
 or ensuring that if you are ever unable to speak for yourself due to illne
 ss or injury\, someone you trust can make decisions in alignment with your
  values\, beliefs and preferences.</p>\n<p>The person who can make decisio
 ns for you in the event that you are unable to do so is a substitute decis
 ion-maker (SDM) or Power of attorney (POA).&nbsp\; This is someone you hav
 e chosen or who has been legally designated to act on your behalf when it 
 comes to healthcare decisions.</p>\n<p>Having a conversation with your SDM
  about your health care wishes is essential to ensure they understand your
  values\, beliefs and preferences. It's important to be clear about things
  like:</p>\n<ul>\n<li>What types of treatments you would or would not want
  (e.g.\, life support\, resuscitation)</li>\n<li>How much weight you place
  on quality of life versus prolonging life</li>\n<li>Any religious or cult
 ural beliefs that might influence your decisions</li>\n<li>Any specific in
 structions\, such as preferences for palliative care or organ donation</li
 >\n<li>If you haven't had this conversation yet\, it&rsquo\;s a great idea
  to do so\, and to make sure all relevant documents are completed and acce
 ssible.</li>\n</ul>\n<p>By participating in this program\, you acknowledge
