 ilot program (with the intention of extension) that partners with Chelsea 
 School District\, Washtenaw County Community Mental Health\, Chelsea Polic
 e Department\, Chelsea Trinity Hospital\, Chelsea District Library\, Chels
 ea Senior Center\, 5 Healthy Towns Foundation and others countywide to pro
 vide access to follow-up support and resources for individuals and familie
