\nThe Community Social Worker position was created to help bridge the social service gap in\, and on behalf of\, the Western Washtenaw Coun ty community. The position is a 2-year pilot program (with the intention o f extension) that partners with Chelsea School District\, Washtenaw County Community Mental Health\, Chelsea Police Department\, Chelsea Trinity Hos pital\, Chelsea District Library\, Chelsea Senior Center\, 5 Healthy Towns Foundation and others countywide to provide access to follow-up support a nd resources for individuals and families.
