 do so in the near future\, this name change clinic is a great resource to 
 stop by\, ask questions and learn about the process.</p>\n<p>If possible\,
  please register with FreeState Justice in advance of the event by submitt
 ing an intake service request form. Attendees should note in the intake fo
 rm that they will be participating in "Calvert Library&rsquo\;s Name Chang
 e Clinic." In order to register\, you will need to provide a Birth Certifi
 cate or a State-Issued ID and provide a Maryland address where you are com
