 bridge the social service gap in\, and on behalf of\, the Western Washtena
 w County community. The position is a 2-year pilot program (with the inten
 tion of extension) that partners with Chelsea School District\, Washtenaw 
 County Community Mental Health\, Chelsea Police Department\, Chelsea Trini
 ty Hospital\, Chelsea District Library\, Chelsea Senior Center\, 5 Healthy
  Towns Foundation and others countywide to provide access to follow-up sup
