 to get an annual flu vaccine and all children and adults who choose protec

 ">consent form</a>&nbsp\;is&nbsp\;required for all minors ages 6 months th
 rough 17 in order to be vaccinated.</li>\n<li>Youth ages 16 and 17 years o
 f age should be accompanied by their parent or legal guardian if possible.
