I understand that there are certain risks of injury inherent in the pract
ice and play as well as other related activities incidental to my child&rs
quo\;s participation\, and I am willing to assume these risks on behalf of
my child. I hereby certify that my child is fully capable of participatin
g in this program and that my child is healthy and has no physical or ment
al disabilities or infirmities that would restrict full participation in t
hese activities\, except in the "physical limitations" question.
In addition to giving my full conse nt for my child&rsquo\;s participation\, I do hereby waive\, release and h old harmless the Somerset County Library System\, its officers\, sponsors\ , supervisors and representatives for any injury that may be suffered by m y child in the normal course of participation in this program \;and the activities incidental thereto\, whether the result of negligence or any other cause.
