As the pa
rent or legal guardian\, I hereby give my full consent and approval for my
child to participate in this program.
I understand that there
are certain risks of injury inherent in the practice and play as well as
other related activities incidental to my child&rsquo\;s participation\, a
nd I am willing to assume these risks on behalf of my child. I hereby cert
ify that my child is fully capable of participating in this program and th
at my child is healthy and has no physical or mental disabilities or infir
mities that would restrict full participation in these activities\, except
in the "physical limitations" question.
In addition to giving my full consent for my child&rsquo\;s participation\, I do hereby waive\, release and hold harmless the Somerse t County Library System\, its officers\, sponsors\, supervisors and repres entatives for any injury that may be suffered by my child in the normal co urse of participation in this program \;and the activities incidental thereto\, whether the result of ne gligence or any other cause.
