Onsite social worker will provide open hou rs to meet with customers and connect them to the community resource that best fit their needs. During the consultation\, a needs assessment and bri ef history of the presenting problem will be taken and evaluated. Referral s to community or social service agencies will then be provided. Community resource referrals will likely include helping with issues such as: those whom are experiencing homelessness\, mental health or addictions\, health care\, immigration\, veteran&rsquo\;s support\, unemployment\, youth serv ices and senior services. \;
\nCommunity Resource Hour will take place from 5:30 pm to 7:30 pm the fourth Wednesday of each month and meet ing times will be limited to 15-20 minutes as to accommodate other custome rs.
