  clinics \, navigation and connection to essential community resources\, e
 ducation and outreach\, targeted food assistance\, mental health supports\
 , and the BPHN Providers which connect residents across the Black diaspora
  with culturally specific medical providers and specialists within their o
 wn communities to reduce barriers to medical care and treatment.</p>\n<p>C
